Title: Wellness Coach
Program(s): CORE Howe & CORE X Street (Sacramento, CA)
Schedule: Part-time non-exempt, 20 hours/week, Monday-Friday, 1:00pm-5:00pm (must be open to working Saturdays in the future which will substitute one of the other week days)
Pay rate: $16.

Position Description:
The Wellness Coach will serve as a member of a supportive services team for Hope Cooperative’s Wellness Center.
The Wellness Center makes critical differences in our members’ lives.
Members have shared that making social connections has reduced the time they spend in the Hospital and that they have fewer suicidal ideations or attempts.
Members say they want to be here, engaged in positive activities, rather than activities that could jeopardize their sobriety and stable mental health and possibly lead to incarceration or hospitalization.
Members have shared that without Wellness Center they have no one who understands them, no support, and often no established friendships.
Working with members to find employment and volunteer positions in the community has impacted member's desire to achieve self-efficacy and empowered members to explore hobbies, and healthy ways to be entertained, and engage in meaningful relationships.
Essential Duties and Responsibilities: (includes the following; other duties may be assigned)
Support guests, clients, and/or family members with active listening, demonstration of empathy, and carry a message of hope and recovery and empower guests in meeting their identified goals.
Plan and Coordinate activities.
Facilitate classes, activities, and Community Meetings.
Observe, assess, intervene, and refer (within one’s scope of practice) in situations with guests regarding mental health symptoms, physical health symptoms and substance use issues.
Assist in linking guests and/or family members with resources in the community.
Assist guests in developing and following a wellness plan unique to their needs and desires.
Assist guests with various independent living skills: Cleaning, food preparation, hygiene, budgeting money, and safety.
Provide/Assist the guests with transportation in your own personal vehicle as necessary.
Provide positive public relations, community referrals, information, and support by linking guests to needed resources; advocate with and on behalf of guests and significant support persons the guests have identified.
Facilitate planning of social, cultural, educational, supportive, and recreational group activities.
Act as a designated Mandated Reporter for the observed or suspected abuse and/or neglect of children, disabled individuals, dependent adults, and elder/senior citizens.
Mandated Reporter also includes “Duty to Warn” which includes the responsibility to notify authorities and the intended victim(s) of physical harm.
Regular and predictable attendance is required.
Use motivational interviewing, trauma-informed care, and harm-reduction practices
Education and Experience:
Lived experience as a mental health consumer or family member of a mental health consumer
Work experience with Medi-Cal billing is highly desirable
Peer Certificate is highly desirable
Work experience with persons who have substance abuse problems.
preferably individuals who are dually diagnosed (mental health & substance abuse) is highly desirable
Knowledge of 12-Step Programs and/or personal experience with substance abuse recovery is highly desirable
Certificates, Licenses, Registrations:
Possess a valid California Driver’s License and current vehicle insurance.
Driving records must meet, and continue to meet, the underwriting standards established by the generalized driving guidelines used by our insurance broker.
Must provide proof of insurance.
Must have at least 3 or more years of driving experience.
Other Skills, Abilities, and Job Requirements:
Ability to pass clearance of the Federal Bureau of Investigation (FBI) and/or Department of Justice (DOJ) background screenings.
Consumer of mental health services or a family member of a mental health consumer is desirable.
