Job Summary:
Supervises a team of community health workers (CHWs) who will be conducting homebased and telephonic visits with patients, assessing community and social support needs of patients/caregivers, and helping with patient action plans.
Motivates staff to improve their performance continually.
Provides direction to staff and evaluates the level of care provided.
Job Requirements:
Education and Work Experience:
Bachelor's Degree or equivalent combination of education/related experience: Required
Master's Degree: Preferred
Five years' relevant experience: Preferred
One year leadership experience: Preferred
Successful completion of a Community Health Worker formal training program from an accredited college, or other educational institution: Preferred
Licenses/Certifications:
Valid Driver’s License (DL) and must be at least 21 years of age or older: Required
Essential Functions:
Acts as an expert resource to the CHWs, assisting in trouble-shooting and brainstorming options to assist with the coordination of care and services with community team members.
Completes supervisory and urgent home visits with CHWs.
Audits, coaches, and holds CHWs accountable to metrics.
Assigns work to staff.
Validates and signs off on CHW documentation.
Documents all patient encounters (in-home, telehealth, and telephonic) per documentation standards.
Participates in multi-disciplinary team meetings and clinical case conferences as required.
Coordinates and communicates with health plan case managers, facility staff and others involved in patient care.
Acts as a patient advocate and liaison between the patient/family and community service agencies.
Maintains Health Insurance Portability & Accountability Act (HIPAA) compliance at all times.
Interview, Hire, Train CHWs.
Directly lead and supervise CHWs.
Provides direct support to CHWs.
Assists the CHWs to coordinate social and health care between member and the health and community organizations.
Support CHWs to assess and identify barriers to care; determine goals, objectives, and potential alternatives to care to develop an integrated care plan.
Holds team meetings.
Provides regular CHW training.
Assists with public presentations.
Communicates closely with other healthcare providers so that primary and specialty care is well coordinated, including the use of virtual consult platforms.
Ensures providers are aware of any urgent needs.
Performs other job-related duties as assigned.
