*Position Summary:*
As a Certified Nursing Assistant (CNA) working the PM shift at Lincoln Meadows, you play a crucial role in providing high-quality care to our residents during the evening hours.
You will work as part of a dedicated team committed to delivering compassionate and personalized care in our facility.
*Key Responsibilities:*
*Resident Care:* Provide direct care to residents in accordance with Lincoln Meadows' established policies, procedures, and individualized care plans.
This includes assisting with activities of daily living, such as bathing, grooming, dressing, and toileting.
*Observation and Documentation:* Observe and record changes in residents' health status, vital signs, and any other relevant information.
Promptly report any concerns or observations to the nursing staff.
*Safety and Comfort:* Create a safe and comfortable environment for residents.
Help with transferring and positioning residents safely to maintain their well-being.
*Communication:* Maintain clear and effective communication with the healthcare team, residents, and their families.
Show empathy, compassion, and attentiveness to resident needs.
*Rehabilitation Support:* Collaborate with licensed therapists to support residents' rehabilitation programs, which may include assisting with exercises and mobility activities.
*Team Collaboration:* Work closely with nurses, therapists, and other healthcare professionals to ensure coordinated and comprehensive care for residents.
*Infection Control:* Follow infection control protocols and procedures to minimize the risk of infections within the facility.
*Qualifications:*
* Current certification as a Nursing Assistant (CNA) in the state of [State Name].
* CPR certification preferred.
* Previous experience in a skilled nursing or long-term care setting is advantageous.
* Strong interpersonal skills, empathy, and a commitment to providing compassionate care.
