*Care Manager*
*Job Qualifications*
· Experience in care coordination, case management, or clinical practices
· Three plus years of experience as a case manager or 1 year of similar duties
· A valid Driver's License, reliable daily vehicle, and active CA auto insurance required
· Medical terminology or background in healthcare
· Experience working with individuals with complex conditions which may include chronic physical health issues, mental illness, substance use disorders, homelessness, justice involvement and the frequent use of multiple health systems.
*Job Duties*
· Interview patients to identify their medical and social service needs
· Educate patients about their condition, medication, and give specific instructions
· Develop a care plan to address their personal health care needs
· Respond to patient's questions and expressions of concern
· Comply with hospital and legal requirements by fostering nursing practices that adhere to the hospitals and nursing division's philosophy, goals, and standards of care; requiring adherence to nurse practice act and other governing regulations
· Work as part of a team with other members of the interdisciplinary team
· Facilitate the coordination of social services for patients
· Perform other duties as assigned
*Education*
Minimum of a bachelor's degree in an appropriate field of study
