On an average day you will:
Spend quality one-on-one time with patients and their families to support everyday needs
Care for hospice patients by providing paths, back rubs, oral hygiene, shampoos, and changing bed linen as often as needed
Help patients dress and undress
Record important health information including temperature, pulse, respiration, and blood pressure
Assess and report the patient condition and any significant changes to the Case Manager
Assist patients with ambulation and exercise
And additional duties as assigned
Qualifications
An enthusiasm for providing the best possible care to patients and their families
A CNA and Home Health Aide certification in good standing
1 year of experience in hospice or home health preferred
A valid driver’s license with an automobile in good working order that is properly registered and insured
Current CPR certification
Excellent observation, verbal, and written communication skills
And additional related duties as needed
