Compensation:
$18.

Monday - Friday from 6:00 am - 2:00 pm/ or 2:00 pm - 10:00 pm

Connections For Life provides custom, tailored support throughout Solano County.
The people we support have individualized hours of support specific to their Individual Service Plan (ISP).
You may apply directly to one city, or you may apply to support all service areas: Vacaville, Fairfield/Suisun City, Vallejo, and Benicia.
Please note: An open availability/applying to all service areas will provide greater opportunity for your preferred shift time(s).
It is Connections For Life’s policy that all Direct Support Professionals work no less than 8 hours weekly.
Requirements:
· High school diploma or GED equivalent
· A minimum of 18 years of age.
· Valid Drivers License
· Proof of Auto Insurance
· CPR/First Aid Certification (offered internally after 90 days)

Responsibilities:
The Direct Support Professional has the following responsibilities and duties:
Supports individuals with maintaining a safe household.
Adhering to and proving individualized support outlined in the Individual Support Plan.
Accepts and works within established household routines.
Accepts assigned responsibilities for laundry, shopping, meal planning, and preparation, including household cleaning.
Supports with hygiene and daily health needs, including but not limited to, shower assistance, personal care, cleaning, and medication.
Adheres to filling out the Medication Administration Form (MAR) & signing chore lists.
Welcomes and respects the individual’s friends and other visitors.
Assists the individual in developing and maintaining positive relationships in the community.
Maintains appropriate timekeeping reflecting accurate hours worked.
Communicate effectively to appropriate CFL management in the event of accidents as required by Incident Reporting procedures.
Attends routine house meetings, workshops, and training as requested.
Provide reliable transportation to and from appointments, grocery stores, community activities, and events.
Accesses CFL email every week; checking emails, utilizes to communicate necessary information to CFL management.
Accesses CFL scheduling platform every week to ensure the accuracy of services provided.
Direct Support Professionals are responsible for ensuring their schedules are accurate and notifying appropriate CFL management of discrepancies.
Other duties as specified and assigned by the employer.
Requirements:
Physical Demands
While performing the responsibilities of the job, the employee is required to talk and hear.
The employee may be required to assist a person in movement from bed to chair or chair to vehicle.
The employee may be required to assist a person from a chair to shower or bed via the Hoyer lift.
CFL has a no-single lift policy, Direct Support Professionals are fully trained and capable of appropriate movement procedures for the people we support, including the usage of Hoyer lifts.
The employee is often required to sit and use their hands and fingers, to handle or feel.
The employee is required to stand, walk, reach with arms and hands, climb or balance, and stoop, kneel, crouch, or crawl.
Vision abilities required by this job include close vision.
