THE ORGANIZATION
Alta California Regional Center (ACRC) is a non-profit agency under contract to the State of California and for 50 years has provided the coordination of services to individuals with developmental disabilities.
ACRC employs approximately 500 persons working out of eight offices in a ten-county area.
ACRC is one of 21 Regional Centers in California.
THE POSITION AND JOB SUMMARY
ACRC is seeking an Early Intervention Service Coordinator reporting to the Roseville office.
The Early Intervention Service Coordinator will have cases anywhere in the outlying 5 counties: Placer, Nevada, Yuba, Sutter and Colusa Counties (NOT Sac County) and will provide case management and advocacy for children from birth to three years of age with a developmental delay, established risk condition, or who are at high risk of having a developmental disability.

Responsibilities include; ensure that the transition process is implemented fully for each child; ensure the details are completed per DDS guidelines; initiate the transition process at 27 months; collaborate with school district representatives; assist parents with navigating insurance for both psychological evaluations and ABA services to review to assist with the teaming eligibility process; SC will prepare referrals for psych scheduling.
Follow up with sending psychologists the therapy reports needed when a psychological evaluation is scheduled, submit the POS, and obtain a signed release of information.
If an interpreter is needed the SC will also make a referral to the interpreter and submit a POS; SC will obtain reports with age scores by the 32-month report.
The scores not included in the report will require the SC to complete additional evaluations virtually or in person with the DP3, IDA or DayC2, gathering age scores from the DP3 for ALL developmental areas to ensure there are no delays following the 32-month report from vendors as has previously been required.
This supports the new Provisional Eligibility option for families; the Transition Specialist/SC will send school districts the late transition referral to the LEA consistently upon eligibility determination; the Transition SC may begin to discuss SDP as part of the discussion related to services; the Medicaid Waiver may be initiated prior to transferring the child to the children’s unit; teaming for Lanterman services on-going requires med records, vendor reports, additional requests to families for genetic/ medical reports and follow up.

Service Coordinator II: $22.
71 - $31.
94/hr.
(must possess a bachelor’s degree and 5 years of regional center experience)
Service Coordinator IIIB: $24.
14 - $33.
96/hr.
(must possess a master's degree and 1-year post-masters experience)
Service Coordinator IIIA: $26.
65 - $ 37.
53/hr.
(must possess a master's degree and 2-years post-masters experience)
Requirements:
JOB REQUIREMENTS AND QUALIFICATIONS
Applicants must possess: a Bachelor’s or Master’s degree in Social Work or a related field; demonstrate excellent written, verbal, and interpersonal communications skills; and have strong time management and organization skills.
One year of professional experience in social work or a related field (preferably in the field of developmental disabilities) is preferred.
