Action Supportive Care Services (ASCS) is looking for a qualified health care aide for direct 1:1 student care in a school setting.
The health care aide will assist with meeting the physical, emotional, health and safety needs of the patients as medically indicated and ordered by the MD.
As well as;
· Demonstrates knowledge of and adherence to Agency policies, Health Aide scopes of practice.
· Demonstrates the ability to act independently, under the supervision of an RN, within your scope of practice.
· Provides safety precautions specific to each patient.
· Maintains confidentiality of Protected Health Information, abiding by HIPAA regulations.
· Notifies the Registered Nurse, and appropriate Agency personnel, in a timely manner, of observed changes in the patient’s condition.
· Concisely and legibly documents patient vital signs, observations, monitoring and care provided according to Agency policy.
· Demonstrates knowledge of proper equipment use and ensures all necessary equipment and supplies are present.
· Demonstrates ability to properly provide for the nutritional needs of the patient.
· Recognizes and honors Patient Rights.
· Maintains a non-judgmental attitude towards patients and their families, and coworkers regarding lifestyle, race, religion, ethnicity, and personal beliefs.
· Consistently practices Universal Precautions and other infection control measures as appropriate.
· Practices safe work habits to protect the patient and the health aide; reports unsafe working conditions.
· Attends appropriate in-services; meets mandated reporting requirements and self-directed learning for professional growth.
· Completes all assigned shifts as scheduled.
Communicates effectively with Agency staffing regarding schedule.
· Appearance is professional, following Agency guidelines.
· Performs health aide duties as assigned by a Supervisor; demonstrates willingness to accept other assignments due to changing school care needs.
Required Knowledge, Skills, and Abilities:
· Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.
· Must be able to communicate with clients, families, school staff, and other health care workers.
Speech clarity, speech recognition and written expression are vital abilities for performing this health aide job.
· Must be able to lift and carry up to 40 lbs.
with or without assistance and must be able to stand, walk, bend, and stretch as required for client care.
