*JOB SUMMARY:*
Under general supervision of the Case Manager Lead, the Case Manager will evaluate the needs of seniors referred to the case management program.
This position applies the principles of whole person care in the assessment, identification of needed referrals, follow-up, and monitoring of interventions.
*Reports to:* Case Manager-Lead, Program Director, Executive Director
*Pay Range:* $20-35 per hour
*Schedule:* Regular, Part-Time or Full-Time

*ESSENTIAL DUTIES:*
* Receives and completes intakes for the Nutrition Program.
* Performs initial Nutrition Program in-home assessments including screenings of prospective clients to determine eligibility for organizational programs and services.
* Performs quarterly assessments based on the Nutrition Program requirements in the home or phone.
* Implements and provides interventions for the PEARLS program as needed.
* Evaluates the effectiveness of referrals and documents all communication in the record.
* Transitions clients who no longer qualify for the program.
* Maintains accurate records of client assessments and concise notes in client case logs.
* Enters client data into the Get Care database and discusses established start dates in collaboration with Nutritional Services.
* Performs data entry for programs using program specific databases.

* Maintain on-going communication with the Nutrition Program clients providing appropriate information and referrals.
* Establish and maintain positive relationships with other senior support service providers and become well versed on eligibility requirements.
* Report any suspicion of elder abuse to appropriate authority.
* Participate in ongoing training to enhance skills.
* Participate in regular community meetings to learn about available services.
* Develops relationships with care coordinators/discharge planners, social workers in local hospitals, skilled nursing facilities and other non-profit senior support services, to identify clients for the Medically Tailored Meals program and Meals on Wheels.
*KNOWLEDGE, SKILLS & ABILITIES:*
* Must possess the ability to relate to older adults, demonstrating sensitivity, active listening, and a respect for self-determination.
* Able to interpret policies, follow procedures, take instructions and carry out assignments to completion.
* Must possess good judgment and good judge of character.
* Ability to establish and maintain cooperative working relationships with staff members, subordinates, administrators and community collaborators.
* Experience computer operator with the ability to learn new applications quickly.
* Must possess good written and verbal communication skills and write legibly.
*MINIMUM QUALIFICATIONS:*
* Associates degree or certification in gerontology, senior health, nutrition, community service, social work, counseling, mental health.
* Trained in Whole Person Care/Options Training and PEARLS at time of hire or within six months of employment.
* 2 years professional experience working with older adults
* Reliable transportation for fieldwork.
* Current California Driver’s License and Automobile Insurance.
* Ability to use a PC or laptop computer with Windows operating system.
* Ability to sit and stand for a long period of time.
* Ability to walk up and down stairs and on uneven ground.
* Ability to lift and carry 30 lbs.
