This Patient Navigator position is for the Case Manager role in the Early Psychosis Programs at the UC Davis Medical Center.
The Patient Navigator participates in weekly clinic team meetings, assists with community outreach and education efforts, provides education and case management support to consumers and their families as part of a clinical team, and assists consumers and their families in accessing community resources.
Services may be provided in a variety of settings, including the clinic, the community, local schools, or the family home.
Through contact with consumers and families within SacEDAPT, the Patient Navigator assists the treatment team in identifying individual, family, home or community conditions needing intervention and support the client and family in meeting those needs.
Also responsible for engaging Non-English speaking clients and family members/support persons and assist in their ongoing care coordination as part of the culturally responsive model of care.
Apply By Date 3/19/2024; Interviews and recruiting process may occur at any time Minimum Qualifications Experience interacting with people of diverse backgrounds, the ability to identify when differences in culture of language may impact communication, and how to resolve those differences appropriately and with cultural humility Bachelor’s degree in related area and/or equivalent experience/training Knowledge of community resources in Sacramento county Previous experience supporting individuals or families in accessing supports or services Excellent oral and written communication skills to effectively work with physicians, consumers and families, clinical staff, research staff, and other health care professionals Demonstrated knowledge and experience with Windows based computers and associated software programs (such as Microsoft Office) Ability to work independently and manage multiple deadlines Previous part-time or full-time work experience Preferred Qualifications Experience working with alcohol and drug services, child welfare, and/or juvenile justice system is strongly preferred Bilingual language skills, oral and written, are strongly preferred Use of Spanish language skills in a professional context Previous experience working with family members of adolescents and young adults with major psychiatric disorders is strongly preferred, with at least 2 years of professional work experience applying those skills Experience with assisting in psychiatric case management services including psychiatric hospitalization and linkage to community resources Experience with public speaking Department Description The goal of the UCD Early Psychosis clinical and research programs is to intervene as early as possible in order to support recovery for youth with serious mental illness.
