Position Details
The outreach and engagement specialist plays a crucial role in facilitating access to essential healthcare services for under served populations.
This position primarily involves monitoring and engaging with potential program participants, coordinating with care teams, documenting program activities, and fostering community partnerships.
The specialist works closely with the Extended Care Management (ECM) team to ensure seamless integration of services and optimal outcomes for program participants.
Duties and Responsibilities:
Monitoring managed care plan (MCP) member information and contacting potentially eligible ECM members.
Assigning enrolled members to appropriate ECM care team members based on risk and clinical data.
Helping the ECM care team engage with unresponsive members and coordinate with accessible settings.
Documenting program enrollments, assessments, and progress notes.
Assisting other ECM care team members with delegated tasks and identifying potential members.
Engaging with community members in vulnerable areas to encourage program participation.
Collaborating with ECM care team members on patient care and case reviews to prioritize healthcare needs and remove barriers to care.
Responsible for forming referral relationships with external stakeholders to create hot hand-offs and closed loop referrals.

Requirements:
Our ideal candidate will possess the following skills:
High school/GED
Active driver's license required
Lived experience of navigating systems of care (examples include: medical, housing, mental health, substance use, regional center, education, etc.
) for self or a loved one preferred.
Two years' experience document activities with community outreach and behavioral health with high-risk populations in a electronic health record preferred.
Bilingual in Spanish preferred
